I consent to the STA using my personal email address to communicate with me regarding the Mentor 36 program. I understand that the STA cannot guarantee the security of email messages that I receive from or that I send to the STA. I understand it is my responsibility to monitor email received at the indicated email address and to advise the STA if the email address changes. I understand that I may stop using this personal email address to communicate regarding Mentor 36 at any time, at which point I will notify the STA in writing of my decision to stop using this email for this purpose. If you have any concerns about your personal email address being collected, used, and disclosed as is reasonably necessary for this purpose, please let us know.
